Disability Discrimination Practice Checklists (DDPCs)

There are currently four DDPCs - one for health professionals, one for service managers (public sector), one for course tutors, one for vocational / employment specialists and assistants.  The need for other checklists (e.g. for child health practitioners) is under discussion.  The DDPCs each comprise 25 items (except Assistant checklist - 20 items) relating to disability under the Equality Act, geared to each of the four broad work roles.   

It is important to read the Introduction to the DDPCs, which provides essential background information including details of authors, copyright, permissions and acknowledgements) before completing the DDPC itself (see downloads below)

The links below will take you to an on-line google form for your chosen practice checklist.  On completion you will receive a copy of your submission.  Prior to completion you will be asked to provide your email address so that you can be emailed a copy of your completed form following submission.  The emails are for this purpose only and will deleted every time the forms are accessed and not be retained on the spreadsheet.
