What personal information might be held.
The website includes various questionnaires which you may have decided to complete and submit. You may also have contacted me through the website and/or by email and provided information such as the following:
• Name and other contact details
• Your current professional and other relevant circumstances
• Any disabilities you have chosen to report, any experiences of disability discrimination and any reasonable adjustments received.
The source and use of any personal data
The purpose of the website is to raise awareness and provide information about disability rights and discrimination including responding to requests for information. If I hold any personal information about you this will most likely have come directly from you. However it is possible that information may have been reported by a family member, professional or colleague who have provided an example of disability discrimination or the provision of reasonable adjustments that you have experienced or been party to. However, I will not have asked for any personally identifiable information
Any personally identifiable information provided via the website or via e-mail will not be disclosed by me to any other person without consent.
Any personally identifiable information received via questionnaires will be collated and reported on website or in presentations collectively. In any individual examples any personal details will be removed or anonymised.

Duty and limits of confidentiality
I have a legal duty of confidentiality. However, professionally, I may be obliged to break confidentiality if, for example, I receive information that suggests that a person may be at high risk of serious harm (e.g. suicide). I may also be obliged to share information in the unlikely event that I am provided with information about to a serious crime. In the event of serious professional misconduct, I may be obliged to raise concerns with the person concerned, their manager/employer and/or with relevant regulatory authorities.
How long information will be kept
Any personally identifiable information reported to me will only be held for the time period for completion of the purposes for which the information was provided to me. It will then be deleted. Emails will be kept during active communication plus any relevant follow-up period. All emails will be deleted following 5 years of inactivity, on retirement from related professional activity or after a request to delete.

How to complain
